Medical Questionnaires

As part of your black belt assessment and grading there will be a medical questionnaire exam, this is as important as the techniques when grading, it will not be acceptable to refer to the bones in you ear as 'Hammer', 'Anvil' and 'Stirrup'.

I found it useful as a in car quiz when driving to and from the clubs, Members of the family would take it in turn to ask or answer them. The list is not exhaustive but will provide good preparation for the black belt exam.

If you are from the Cotgrave or East Leake clubs you can bring you completed questionnaire in for marking, any that pass 80% will receive a certificate of achievement.
